Child Hosting Forms

If you wish to apply to host a child, please download the following forms.  The full instructions for the required Police Checks are in the Potential Host Family Letter.  Please read it carefully as Police checks are different depending upon where you live.  All forms are required for applying to host a child.  The Police Check Forms link below is only for residents of the City of Brockville.

• Police Check Forms  This link is only for residents of the City of Brockville (you will need both the consent form and the vulnerable person form).  OPP forms must be picked up at your local OPP detachment, but first contact Riley for a CAC letter to take with you.

This form is to apply to become a member of Canadian Aid for Chernobyl or to renew your annual membership.

Humanitarian Assistance Application

This form is used by members for humanitarian assistance or sponsoring a child’s education in Belarus.  It must be completed, submitted and approved by the committee for a tax receipt.
